Please forward any special requests that you have and would like for me to present to the Company for consideration by completing the form contained in the following link:

An example of a special request would be a Premises Technician who is not on the list for canvassing but is wanting to exit payroll.

Please complete the form as soon as possible and once submitted, you will immediately receive a confirmation of receipt. There is no further action required. If you do not receive a receipt confirmation, please send an email to svolante@cwa-union.org and Suzanne will confirm if the form was received.
